Before filling out this form, you will need to go online, create a username with the FCC and register to get an FCC Registration Number (FRN). The link to that site is: https://apps.fcc.gov/cores/. You MUST have an FRN to complete this form.

Fill out and submit this registration form and you will be contacted regarding your test session. Be sure to bring one form of photo ID to the test session.

If already licensed and upgrading, you must bring a copy of your current valid license, and/or a valid Certificate of Successful Completion (CSCE) for any upgrade elements already taken.

There is no fee for testing.

It is highly recommended that you make no other application to the FCC (such as applying for a vanity call) after making this reservation.
